Patron applications are available at the Library or you may fill out, print, and bring the application form below to the Library along with the required identification.

Residents of Buckfield, Hartford, and Sumner may apply for a card at no cost. Residents of other towns may obtain a card for a fee of $10 per year. Proof of residence (e.g., driver's license, lease, utility bill, or current tax bill) and a photo ID are required for a library card.

For minors (under 18), the parent/guardian must sign the application. Children are encouraged to have their own Library cards.
